Leviton's telephone jack wall plate is both attractive and convenient. Simply plug in your phone cable jack and optionally mount the phone on the wall using the included lug nuts. Designed for 4-conductor wall mounted phones.

How is this installed?
Hardware bracket behind plastic wall plate mounts into electrical box in box mounting screw hole slots that are a standard 3-5/16" apart center-to-center. Lug nuts for hanging phone, if needed, mount into recessed holes at top and bottom of plate that are 3-15/16" apart. Plastic plate then mounts into hardware bracket in device mounting screw holes that are 2-3/8" apart.
